Overview

Join to apply for the Nurse Practitioner Band 6 (RMN) role at NHS. This is a community-based mental health post with flexible and agile working, local living, and clear development and career progression opportunities.


Position

Nurse Practitioner Band 6 (RMN)


Location

Kingston Home Treatment Team (KHTT), based at Tolworth Hospital and working across the Borough of Kingston. The post holder must be able to work shifts.


Hours and Salary

Early Shift: 08:30 to 16:30; Late Shift: 14:30 to 22:30. Salary: £44,485 to £52,521 per annum, inclusive of Outer London HCAs. Permanent, full-time.


Responsibilities

* Engage clients in crisis and participate in assessment and treatment.
* Provide a range of bio/psycho/social interventions for service users and carers.
* Plan for robust and sustainable discharge/transfer and aftercare; involve clients in planning and evaluating care.
* Work as part of the team to achieve the best possible care for clients.
* Support clients in developing a Collaborative Crisis Plan.
* Be the first point of contact for all referrals within the Home Treatment Team (CHTT).
* Develop and maintain high standards of practice based on assessment, psycho-education, the Recovery Model, and supportive clinical management.
* Communicate effectively within relevant agencies and teams to ensure integrated care.
* Attend clinical meetings to provide information on caseloads and new assessments.
* Identify and support carers and facilitate appropriate assessments.
* Advise on referrals to HTT or signposting if not appropriate for HTT.
* Undertake home visits (single or joint) when there are no health, risk, or safety issues evident.
* Actively participate in relapse prevention work and support self-management and collaborative crisis planning.
* Provide comprehensive risk assessments in line with Trust Guidelines.
* Ensure KPI data is assessed, recorded, and updated regularly on RIO.
* Engage service users and carers with flexible approaches to care.
* Maintain effective communication with inpatient services, social services, CMHTs, and other statutory/non-statutory organisations to ensure continuity of care.
* Have knowledge of Mental Health Act, Mental Capacity Act, Care Programme Approach, Care Act, Deprivation of Liberty Safeguards, Safeguarding Vulnerable Adults, and Community Treatment Orders.
* Maintain confidentiality and accurate records in line with NMC guidelines and Trust Policy.
* Manage medicines in accordance with NMC guidelines and Trust Medicines Policy.
* Coordinate with Children & Families Teams when safeguarding children.
* Report incidents or Serious Untoward Incidents to the Team Manager.


Essential Qualifications and Experience

* RMN with current NMC registration.
* Evidence of continuing professional and personal development.
* Full and current driving licence and car ownership (or willingness to participate in the Trust car lease scheme).
* Post-qualification demonstrable experience of working with people with serious ongoing mental health problems in a community or outpatient setting.
* Experience of working in a multidisciplinary team.
* Experience of assessing and managing crisis in severe mental illness.
* Experience of working as a clinical supervisor (desirable).


Desirable Qualifications and Experience

* Mentorship, ENB 998 or equivalent.
* RGN.
* Diploma in cognitive behavioural therapy/psychodynamic counselling.
* Experience of managing and supervising staff.
* Experience contributing to policy and procedure development and implementation.
