Job Title: Midwife - Diabetes in Pregnancy Service
This post has been established to support the seamless and effective running of the diabetes in pregnancy service.
The midwife will work with the multidisciplinary diabetes in pregnancy team in both face-to-face and virtual settings, harnessing technology to support women and their families diagnosed with gestational diabetes.
The post holder will be required to utilise the GDm-Health app, onboard eligible women, and support them in its use. They will follow up with women according to local and national diabetes guidance, support engagement with appropriate services, and assist women in making informed choices and preparing for parenthood.
The clinical responsibilities include supporting the diabetes antenatal clinic, the virtual diabetic clinic, and managing the GDm-Health app dashboard. The post holder will also be responsible for auditing the service and maintaining/updating the database.
The post holder is expected to support research projects related to diabetes in pregnancy.
The midwife will work in partnership with the lead midwife for the service, supporting women to achieve positive pregnancy outcomes that are personalized, safe, and evidence-based.

Key Responsibilities
1. Work with clinical and non-clinical staff to improve patient and staff experience by reducing waiting times and overbooking.
2. Communicate complex information regarding diabetes in pregnancy to patients.
3. Effectively communicate with staff involved in the antenatal and diabetes services.
4. Provide midwifery support for the diabetes in pregnancy service.
5. Identify and explain the use of the GDm-Health app to eligible women, monitoring usage via the dashboard.
6. Organise follow-up visits and triage women onboarded onto the GDm-Health app.
