Themain purpose of the Band 6 Nurse is to provideassessment planning and implementation of health care for adultsand young people attending the Foundation. To be responsible forsupporting adults and young people through medical needs bothphysically and emotionally. Nurses work and collaborate with otherprofessionals to ensure there is seamless support to adults youngpeople andfamilies.
Themain duties of the Band 6 Nurseare:
Toplan implement and evaluate nursing interventions and healthpromotionaldevelopments.
Tomake referrals or liaise with appropriate hospitalspecialties/agencies.
Tosupport staff in providing up to date and skilled care which willinclude the administration of medication passing of nasogastrictubes care of tracheostomies gastrostomies and the passing ofcatheters administration ofoxygen.
Tooversee the development of policies and procedures for medicationincluding MARsheets.
Tobe able to work effectively within a day service and residentialsetting.
Support withrelevant auditingactivities.
Toattend annual review meetings discharge planning meetingsmultidisciplinary meetings and caseconferences.
Tobe part of the assessmentprocess.
Tobe responsible for delivering or arranging training ofstaff.
Toparticipate in the training programmes creating an appropriatelearning.
Tomaintain accurate professional records in accordance withNMC.
To ensure medical care plans are updated and sharedwith all servicesinvolved.
Toensure all Safeguarding incidents are reported in line withpolicies andprocedures.
Toinvolve parents and young people and encourage them to makeinformed choices concerning their health and generalwellbeing.
Tocommunicate effectively with the young person and their carers tounderstand their condition. To give sensitive/emotive informationaround diagnosis and/or outcomes.
Theknowledge and experience required of Band 6Nurseare:
Asound knowledge of diabetes cerebral palsy muscular dystrophy andautism
Asound knowledge of tracheostomies gastrostomies and seizuremanagement
Experience ofworking with individuals with learningdisabilities
Anunderstanding of national initiatives (i.e. LeDer STOMP YourWelcome)
Moving andHandlingskills
Ability tocommunicate and collaborate withcolleagues
Beinvolved in making shareddecisions
Qualified andregisterednurse.
