Salary: Pay Grade C - £24,546.49 to £26,294.81
Hours: 37 hours per week, Monday to Friday with occasional evenings and weekends evenings. Happy to discuss flexible working.

Place of work: Based 3 days in CAMHS Southampton and 2 days at No Limits base / in the community

Your Role

As a Social Prescriber, you will work closely with CAMHS regarding the emotional wellbeing and mental health needs of children and young people, providing a link between the voluntary sector and CAMHS.

Operating three days per week at the Southampton CAMHS Wellbeing Hub, you will triage referrals and gather essential information to facilitate assessments and ongoing support for children and young people both within CAMHS and back into the community.

For referrals which would benefit from community-based support, you will spend two days per week working within a social prescribing capacity in the community, connecting young people to local services and offering support to engage with those services.

Social prescribing aims to forge connections to community groups, educational opportunities, employment resources, and statutory services for practical and emotional support.

Your key responsibilities will include:

* To work alongside CAMHS and other statutory/voluntary organisations in the Wellbeing Hub providing telephone triage and reviews for all incoming CAMHS referrals and CAMHS patients
* To act as the first point of contact through referral and signposting for young people to the appropriate service, internally and externally, based on their needs.
* To provide an assessment / review to determine the young person’s requirements for help and support and what services might be best placed to provide this.
* To provide, or signpost to, brief interventions for a range of healthy behaviours such as physical activity, smoking cessation, healthy eating, emotional health and wellbeing, health related school attendance issues, sexual health, behaviours and relationships and other lifestyle related matters.
* To engage with referral agencies, MHST’s, student welfare services, youth and community services, primary health services, mental health services, youth offending teams and community police officers to share information about, and promote referrals to, the service.
* To build effective working relationships with statutory services and third sector agencies to provide a holistic, person centred approach to service delivery
* Provide quality up to date information, advice and support which taps into personal, health and social development in a variety of local settings/environments.

You’ll have the essentials of:

* Proven experience of working in a range of settings and environments with children and young people with a range of levels of need and risk, with an ability to effectively manage your own wellbeing.
* Experience of assessing risk and implementing safety procedures when thinking about children and young people’s emotional and mental wellbeing or those at risk of violence or exploitation.
* Experience of case holding clients and maintaining individual client files.
* Experience of successfully engaging young people with challenging behaviours and/or complex needs, young people who may be reluctant to discuss their needs or be angry or confused.
* Knowledge of a range of issues, including risk behaviours, substance misuse, mental health, self-harm, family dysfunction, gangs, county lines
* Able to build trusting relationships with young people, enabling them to develop new skills and make their own informed choices.
* Demonstrate effective organisational skills and good time management skills.
* Demonstrate the ability to work and communicate effectively as part of a large multidisciplinary team.

You may also have:

* A Mental Health, Youth Work, Social Work, or equivalent qualification at level 4, or equivalent experience.
* Experience of working in a multi-disciplinary team and of collaborative work with internal and external colleagues which delivered improved outcomes for children.
* Knowledge and experience of developing and maintaining contacts with other agencies/organisations.
* Demonstrate the ability to produce reports, maintain records, statistics and receipts.
