About the Role
The North Herts Integrated Community Team at Hertfordshire Community NHS Trust is looking for a Band 3 Health Care Assistant. The role involves supporting nursing and therapy staff to deliver safe, high‑quality care to patients in the community.
Responsibilities
* Provide assessment and care service to patients with nursing / health needs.
* Visit patients in their own homes and provide appropriate care and support as defined in the Care Plan, including personal care, hygiene, uncomplicated nursing procedures, exercise, activity and treatment programmes prescribed by therapists.
* Rehabilitate and maximise the patient’s potential and where appropriate support the carer.
* Support and enable patients in a range of activities to promote independent living and monitor their well‑being.
* Undertake administration and clerical tasks associated with the care process, including record keeping and use of the computer.
Qualifications & Skills
Excellent communication skills and the ability to work collaboratively with a diverse team of nurses, paramedics, physiotherapists, occupational therapists and technical instructors are essential. Candidates should be motivated and enthusiastic and capable of working flexible hours (8 am – 10 pm, 365 days a year).
Requirements
A full valid driving licence and access to a motorised wheelchair or cart for business purposes is essential (unless an accommodation is available under the Equality Act 2010).
