To lead the Healthy Families Team in the delivery of the 0-19 Healthy Child Programme and Public Health Nursing Programme children in line with local and national public health priorities.

1 x full time post based in Mansfield & 1 x full time post based in Ashfield

Mansfield Healthy Families Team, provide a quality service to the local population, we are an integrated service, working in partnership with local Children’s Centres, working with children, young people aged 0 – 19 and their families .

The team is a skill mixed team, taking leadership from Specialist Public Health Practitioners, providing a high standard of care, our staff receive a good training and induction package and a friendly supportive working environment.

The role includes services to antenatal women, completion of the Healthy Child Programme for under 5s, enhanced programmes of care within school and community settings for the 5 -19 age group, breast feeding support and a strong element of leadership to the skill mix 0-19 cluster team

A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment.

You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.

The post holder, taking the lead for the Healthy Families Team, will be responsible for the case management, assessment, triaging, planning and delivery of care for parents, children and young people using evidence based interventions. This will be delivered in line with the National and
Local guidelines and public health priorities.

You will be responsible for the operational overview of the Healthy Families Team allocated to a defined geographical population. The team will be working proactively using a partnership approach to promote health and contribute to tackling health inequalities within the level of
competencies required for the post.

You will work with children, young people and families within the home environment, school or other identified community setting.

A key aspect of the role is to work flexibly to meet the needs of the service as required.
