What’s on offer:
1. £25,000-£29,000 salary OTE £31,000.
2. 28 days’ annual leave.
3. Weekend on-call only (1 in 6 weekends at £145).
4. Team bonus (up to £1k per annum).

Care Coordinator Responsibilities:
6. Managing and scheduling rotas for a team of 40 caregivers using digital systems (e.g. Access People Planner) to ensure consistent and effective care delivery.
7. Responding promptly to last-minute care changes or emergencies to minimise disruption for service users.
8. Communicating professionally with clients and their families to ensure needs are met and concerns are addressed.
9. Liaising with pharmacies to ensure timely and accurate medication delivery.
10. Supporting the Registered Manager with client assessments and risk assessments to maintain safe, person-centred care planning.
11. Building strong relationships with care staff to promote high standards of service.
12. Ensuring all documentation and records are accurate, up to date, and compliant with CQC regulations and internal policies.
Care Coordinator Requirements:
13. Proven experience as a Care Coordinator or Field care Supervisor ready to step up within a homecare environment.
14. Strong competency with digital scheduling systems (e.g. Access People Planner).
15. Professional, reliable, and proactive approach.
16. Full UK Driving Licence.
