We are pleased to announce two Mental Health Practitioner posts to join our St Helens Recovery Team.
Key Responsibilities
Deliver high quality care and hold a caseload of service users, collaborating with other disciplines to provide comprehensive assessments, advice, and treatment.
Coordinate the care of service users, carrying out assessment planning, implementation, and evaluation under the Care Programme Approach and line manager supervision.
Develop care plans that meet the assessed needs of individuals, ensuring that unqualified staff do not deliver care as identified in the plan.
Liaise with family members, carers, and significant others during assessments and care planning.
Undertake and supervise risk assessments, formulating risk management plans for service users under your care.
Ensure Safeguarding Children and Adults requirements are followed and participate in relevant meetings.
Work collaboratively with other services, external agencies, families, and carers to identify personalised goals and tailored support for recovery.
