Job overview

An exciting opportunity has arisen to become a much-valued member of the Dorset County Hospital’s Acute Hospital at Home service. This dynamic and innovative service provides acute level treatment to patients in the comfort of their own home; Enabling them to be surrounded by the people, routines and comforts which allows them to recover quickly.

The service was established in 2013 and has grown rapidly in recent years driven by innovation, technology and a passion for care and treatment outside of traditional ward settings. The service provides a ‘best of both worlds’ whereby patients can receive hospital level diagnostics, specialist input and flexibility to changing medical circumstances whilst patients are in the comfort of their own home. The role will facilitate effective, collaborative nurse-patient relationships which empower patients and their families to be at the centre of acute treatment.

The successful applicant will work alongside a diverse range of hospital specialties and professionals from cardiology, palliative care, respiratory, frailty, orthopaedic, surgical, acute medical, emergency teams and more. Typical treatments for patients include acute monitoring of vital signs, symptom management and assessment, venepuncture, acute wound management and the intravenous administration of medication such as diuretics, antibiotics and fluids – all in the patient's own home.

Please note that the ability to drive a car is essential for this role.


Main duties of the job

* Access, plan, implement and evaluate individual patient care in patient’s usual residence
* Communicate & work effectively with patients, carers, and the multidisciplinary team.
* Co-ordinate patient centred care from admission to discharge
* Maintain accurate patient records
* Act as patients’ advocate
* Promote the health and empowerment of patients
* To ensure that the patients changing needs are identified and changes initiated or communicated to others
* Follow trust policies and procedures i.e. Risk reporting
* Contribute to the development of the ward/department by attending ward/department meeting and supporting the implementation of change.
* To act as a resource link nurse for the ward.
* Develop oneself and contribute to the development of others

Education, Qualifications & Training


Essential criteria

* To have a degree in registered nursing
* Registered with NMC
* Evidence of other relevant and recent CPD/ training.
* Membership of relevant special interest group.


Knowledge & Experience


Essential criteria

* Minimum of 2 year’s post registration experience in the following areas: General Medicine & Surgery, Orthopaedics, Elderly Care, A/E, Critical Care, Respiratory


Desirable criteria

* Experience of working in the community
* Demonstrate an understanding of professional nursing issues and the 6 C’s


Skills & Abilities


Essential criteria

* Evidence of ability to practice as competent autonomous practitioner.
* Demonstrate an understanding of confidentiality
* Ability to work as part of a multidisciplinary team
* To have a full UK driving licence


Desirable criteria

* Evidence of ability to maintain effective working relationships with colleagues, patients and visitors
* Evidence of an ability to manage time effectively
* Understanding of clinical governance and its requirements especially risk management
