Halton OOH District Nursing Service is seeking a committed and enthusiastic individual to join their team as a community staff nurse, covering the night service.
Applicants must have excellent interpersonal and communication skills, a patient-centered approach to care, be highly motivated, and able to work autonomously as well as part of a team.
The district nursing service provides nursing care to the housebound population of Halton, including patients in their own homes and residential care homes. We also offer a treatment room service for ambulant patients and a comprehensive range of clinical skills.
You will work under the supervision of the District Nursing Sister, delivering:
1. Administration of IV medication/Medicines management
2. Health Promotion
3. End of life/supportive care
4. Holistic assessments
5. Patient-centered care
6. Complex wound management
The service is committed to supporting personal development and offers a supportive 4-week supernumerary period for all new starters.
Main duties of the job
The successful candidate will perform holistic assessments of patient nursing needs, plan, assess, implement, and evaluate care delivery according to changing health needs, utilizing prescribing skills.
Ensure practice is evidence-based by undertaking audits and critically appraising literature to maintain a high standard of care.
Liaise with the District Nurse team leader to ensure effective communication regarding patient care, changes to care plans, and the review of complex care packages.
Engage in mentoring, teaching, and assessing.
Liaise with all healthcare professionals and other agencies to ensure effective service delivery.
Involve patients and carers in the planning and delivery of care.
Maintain accurate records in accordance with NMC standards.
Participate in health needs assessments to identify unmet needs within the community.
Work independently and problem-solve as required.
Work on a rota basis to facilitate weekend working across the district nursing teams.

Qualifications
o NMC registered adult nurse
