An exciting opportunity has arisen for a Band 6 Physiotherapist to join our Assessment and Treatment service, providing sound assessment and treatment to patients with rehabilitation needs. This service is part of the wider Neuro Outpatient group that offers specialist neurological interventions across the pathway.
You will deliver specialist physiotherapy services to patients diagnosed with frailty, stroke, or neurological conditions in various community settings, using developed specialist practices underpinned by theoretical knowledge.
You will serve as a specialist resource for health and social care staff, patients, families, carers, and external agencies, offering advice, support, and guidance in managing stroke and neuro conditions.
Responsible for transferring ongoing patient care to appropriate agencies, ensuring services are conducted professionally, efficiently, and effectively.
Main duties of the job
You will provide effective, evidence-based assessment and treatment to patients referred to the service with rehabilitation, stroke, or other neurological conditions.
Work as part of a multidisciplinary team, ensuring effective communication to maintain high standards of clinical care.
Operate within a clinic environment and undertake home visits as needed to support patient rehabilitation.

Job responsibilities
* Conduct comprehensive, holistic assessments for patients with complex presentations, utilizing investigative and analytical skills.
* Apply advanced clinical reasoning to develop effective rehabilitation plans, seeking guidance from senior physiotherapists when necessary.
* Implement evidence-based treatments and evaluate outcomes using standardized measures.
* Document all activities and patient information on the RiO system using a mobile device.
* Maintain professional and legal responsibility for all aspects of your activities, adhering to practice codes and guidelines.
* Demonstrate physical capability to perform assessments and interventions, including manual physiotherapy techniques.
* Respond to unpredictable work patterns, maintaining high concentration levels during patient contact and other tasks.
* Communicate effectively with patients, carers, and other involved agencies.
