Rutland County Council is seeking an Integrated Support Worker to support coordination of care and support for people receiving services from Micare, with a particular focus on those receiving support from multiple Health and Social care services, such as those at the end of their life.
Responsibilities
* Act as a single point of contact for the person receiving services, removing duplication and ensuring the person only tells their story once.
* Liaise with a multi-disciplinary team across Health and Social care services to bring together a range of support to one place.
* Support coordinators with implementing care and support across all pathways, including Crisis Response, Reablement, D2A, C2A, safety net and complex care. This includes first-visit assessments which require availability during working hours to go into the community on short notice.
* Cover calls for Micare if needed and attend meetings both face to face and virtually when required.
Qualifications
* Experienced social care worker passionate about end‑of‑life care, empathetic, caring and responsive to people’s needs, wishes and preferences.
* NVQ 3/QCF in Social Care or willingness and ability to achieve within an agreed time frame.
* Flexibility in working patterns to meet the needs of people receiving support, including weekend work every other week.
* Ability to work from various locations across Rutland: People’s homes, Rutland Memorial Hospital, Uppingham Surgery, Catmose House and Oakham Enterprise Park. Additional locations may be included as per service development.
* Full driving licence and willingness to travel around the county.
* Successful DBS check (enhanced) – cost paid by the council.
