The Corner Surgery has an exciting opportunity for a Care coordinator / Receptionist to join our friendly, welcoming, and supportive team. We are seeking a hard-working person, who is committed to and has a genuine interest in helping patients, and who enjoys contributing to the delivery of high-quality health care.
We are looking for someone to work all day Tuesday and Friday, with the flexibility to cover holidays as sick leave as and when required.
Job Summary
Main duties of the job include:
* Answering the telephone promptly and dealing with queries appropriately.
* Booking appointments and home visits, ensuring that patients are offered the most appropriate appointment for their need.
* Signposting patients to more appropriate services when necessary.
* Cancelling and rearranging appointments when necessary.
* Processing requests for prescriptions and liaising with the pharmacy team.
* Dealing with patient requests regarding results of diagnostic tests.
* Being the primary link with the social prescribing team, to help co-ordinate the care for patients where non-medical issues may be impacting on their health and well-being, and thus access person-centred local support and education.
* Having the flexibility to be able to cover reception when required during times of annual leave or sick leave.
Care Coordinator duties
* Maintaining and monitoring the practice appointment system.
* Processing personal, telephone and e-requests for appointments.
* Answering incoming phone calls, transferring calls or dealing with the caller's request appropriately.
* Signposting patients to the correct service.
* Initiating contact with and responding to, requests from patients, team members and external agencies.
* Co-ordinating and organising the Patient Participation Group; liaising with the group members and ensuring the agenda and minutes are completed.
* Processing and distributing incoming and outgoing mail.
* Actioning incoming emails when necessary.
* Completing opening and closing procedures in accordance with the duty rota and maintaining security in accordance with Practice protocols.
Receptionist / Administrator duties
* Recording and maintaining accurate patient information, summarising and coding information onto the patients notes using the practice computer system.
* Checking, scanning and electronically filing correspondence from the local hospitals and community services.
* Co-ordinating and organising the Patient Participation Group; liaising with the group members and ensuring the agenda and minutes are completed.

Person Specification
Essential
* Flexibility to work outside of core office hours.
* Disclosure Barring Service (DBS) check.
* Experience of working with the general public.
* Educated to GCSE level or equivalent.
* Excellent communication skills (written and oral).
* Strong IT skills.
* Clear, polite telephone manner.
* Competent in the use of Office and Outlook.
* Effective time management (Planning & Organising).
* Ability to work as a team member and autonomously.
* Good interpersonal skills.
