We offer a relocation package of up to £8,000
The relocation package is for anyone who is joining CWP in a clinical role at Band 5 or above, who would need to relocate in order to take up employment with Cheshire and Wirral Partnership. Terms and conditions apply.

To be part of a team to roll out a new initiative working with young people and families and the wider system to prevent hospital admission.

This is a pilot project named the NOOK, the role will include developing and rolling out the service.

To work as part of a specialist children and young people’s mental health team to provide intensive Tier 4 support and interventions for young people and their families.

As a Mental Health Practitioner you will be responsible for undertaking assessments for young people referred for Tier 4 CAMHS intensive interventions. As well as face to face assessments the role involves all the gatekeeping activities as well as offering consultations to our Tier 3 partners. In addition to gatekeeping activities the post holder will undertake planning and facilitating discharge from Tier 4 services.


Main duties of the job

CWP (Tier 4 CAMHS) provides mental health services for children & young people with emotional, psychological and mental health related difficulties.

The services are provided through assessment for intensive support delivered through a residential inpatient setting or a home based treatment approach.

A whole systems approach is utilised in assessing and offering interventions and support.

To work as part of a team on a rota basis including unsocial hours.
To promote and uphold CWP Trust Values.

Be responsible for undertaking the assessment of young people referred to Tier 4.

To undertake all assessment related activities from screening and triaging referrals, coordinating the assessment, undertaking the assessment, recording the findings and outcome of assessment on the patient record, communicating the outcome of the assessment with all the relevant stakeholders and arranging any interventions offered.

Person specification


Qualifications

* Professional Registration with recognised National Body (HCPC, NMC, Social Work England)
* Nationally recognised post-qualification, continuous professional development (CPD) related to the core profession (Social Work, Nurse, Allied Health Professional).
* Therapeutic Training to intermediate / diploma level


Knowledge and Expertise

* Demonstrable working knowledge of specialist therapeutic approaches e.g. CBT, Family Therapy
* Knowledge of Child Protection workings and Practice
* Working knowledge of Children related legislation
* Experience of inter-agency working
* Experience of writing, implementing and evaluating Care / Treatment Plans
* Experience of working with Students/ Learners
* Experience of providing clinical supervision


Experience

* Significant post qualification experience of working within a CYPMHS or Child Mental Health related field.
* Recognised experience of working within an environment where evidence based interventions have been delivered
* Undertaking formal and informal clinical risk assessments and risk management planning
* Undertaking mental health assessments
* IT skills, including using databases, Microsoft Office and clinical systems
* Post qualifying experience of working within NHS community CYPMHS

You must have appropriate UK professional registration.
