Band 3 Community Mental Health Support Worker - FTB

The Band 3 Support Worker provides high-quality, recovery-focused care and support to children, young people, and young adults experiencing emotional, behavioural, and mental health difficulties. Working under the supervision of the multi-disciplinary team at Birmingham and Solihull Mental Health Foundation Trust, the post holder assists with assessment, monitoring, and delivery of individualised care plans, including therapeutic and group-based activities. The role involves promoting independence, supporting carers, maintaining accurate records, and liaising with internal and external agencies to ensure continuity of care. The post holder is expected to act professionally, follow Trust policies and procedures, and participate in ongoing training, safeguarding, and health and safety practices to ensure the safety and wellbeing of service users and colleagues.


Main duties of the job

Clinical:

Provide care and support to service users under supervision, including therapeutic and group interventions, hospital admission prevention, and post-discharge home support. Promote independence, recovery-focused practice, and health promotion. Monitor daily living activities and service user engagement, providing support to carers and families. Work flexibly, including evenings and weekends, as required.

Administrative:

Maintain accurate and timely written and electronic records to support care planning, monitoring, and evaluation.

Liaise with internal and external agencies to ensure appropriate resources. Report changes in service users' needs promptly and any medication concerns. Promote positive working relationships with colleagues, carers, and partner agencies.

Analysis, Planning & Organisation:

Maintain confidentiality, uphold service user choice and dignity, participate in needs reviews, and attend weekly clinical supervision.
