Senior Occupational Therapist Reablement Service

We are seeking an experienced Senior Occupational Therapist for our new Community Reablement Project within Torbay & South Devon NHS Foundation Trust.
Key responsibilities:
* Supervise and co‑ordinate the Adult Social Care Reablement service Baywide.
* Triage and assess new referrals, identify reablement goals and manage assessments and discharges.
* Deliver direct patient care in community settings, including patients' homes, nursing homes and residential homes.
* Adopt a patient‑centred approach, autonomously decide on packages of care, equipment and treatment plans.
* Maintain accurate and legal documentation, support junior staff and oversee caseload planning.
* Use communication skills to liaise with patients, carers, multidisciplinary teams, external agencies and employers regarding return‑to‑work and service delivery.
* Plan, implement and review OT interventions, including assessment of adaptive equipment, minor/major adaptations, falls and postural management.
* Carry out specialist seating, wheelchair assessments and manual‑handling risk mitigation.
* Develop and contribute to service policy, standard operating procedures and quality improvement projects.
* Engage in clinical audit, research and service development.
* Provide training and supervision to junior staff, students and support workers.
Qualifications and Training:
* Diploma/Degree in Occupational Therapy.
* Valid state registration with Health Professions Council.
* CPD portfolio and evidence of recent post‑graduate learning relevant to the area of clinical practice.
* Experience within the NHS in orthopaedics, elderly care, musculoskeletal and community settings.
Knowledge, Experience and Skills:
* Wheelchair accreditation and pressure‑care knowledge.
* Understanding of complex health and social care legislation and national service frameworks (Older People, Stroke, Falls).
* Strong interpersonal, communication and autonomous decision‑making skills.
* Ability to work under pressure, lead multidisciplinary teams and perform clinical audit.
* Skills in therapeutic, manual handling, risk assessment, hoists, specialist slings and postural management.
* Experience in supervising junior staff or OT students, and participation in clinical leadership.
* Familiarity with Disability Facilities Grants, chronic disease management and community rehabilitation.
* Experience promoting inter‑agency working and initiating workplace improvements.
Additional requirements:
* Disclosure and Barring Service check according to the Rehabilitation of Offenders Act 1975.
* Good working knowledge of national and local policies, guidelines and IT systems.
