Key Responsibilities

* Provide specialist Occupational Therapy assessment, treatment and review for patients with advanced palliative or complex deteriorating neurological conditions in an unstable or deteriorating stage of their condition, supporting patients and their families with complex or rapidly changing needs, predominantly in their place of residence.
* Work as a Band 6 Occupational Therapist within the Community Specialist Palliative Care Team as part of an integrated interdisciplinary team, including Occupational Therapist, Physiotherapist and Therapy Technician.
* Provide a comprehensive range of evidence‑based assessment and treatment to adults in Coventry with a specialist palliative need, delivering care in patients’ own homes or other community settings.
* Follow defined referral criteria to the Specialist Palliative Care Team.
* Liaise closely with team members and other community services to support patients’ effective therapy.
* Provide advice and guidance to patients, carers and staff working in health and social care.
* Support professional development of staff within other services as appropriate.
* Support and contribute toward service development and meeting key performance indicators.
* Assist in the quality and activity monitoring of services.
* Adhere to the lone worker policy.
* Be an integral part of the Community Specialist Palliative Care Team.
* Provide evidence‑based specialist occupational therapy or physiotherapy assessments and appropriate intervention to patients referred to the service.
* Liaise closely with members of the interdisciplinary and wider multidisciplinary team, facilitating effective caseload management.
* Utilise specialist skill and knowledge to manage a palliative care and neurological caseload, addressing the physical, psychological, behavioural, perceptual, cognitive, sensory and environmental needs of patients in an unstable or deteriorating phase of their condition.
* Adopt a patient‑centred and sensitive approach to therapy, collaborating with patients and carers (where appropriate) in all aspects of the therapy process (assessment, planning, intervention, evaluation, reporting).
* Maintain a high level of knowledge of a variety of conditions—including neurological, orthopaedic, musculoskeletal, care of the elderly, respiratory and falls—and understand the impact of rapidly changing presentations.
* Utilise a comprehensive range of evidence‑based specialist assessments and treatment for patients in their own homes or in a clinic environment.
* Assist in quality and activity monitoring systems, supporting evaluation of service effectiveness.
* Demonstrate highly developed communication and motivational skills to facilitate patients in managing their condition.
* Deliver or discuss difficult diagnoses and prognoses with tact, empathy, and while managing distressed or aggressive patients or carers.
* Communicate accurate and sensitive information to patients and carers using appropriate and accessible mediums, considering cultural, gender, age and sensory or learning difficulties, and remaining empathic at all times.
* Respect the dignity and preferences of patients, obtaining consent to therapy and, where required, acting in patients’ best interests when capacity is lacking.
* Provide and receive complex and sensitive information verbally, in writing or electronically to and from consultants, GPs, patients, colleagues and other agencies.
* Attend and participate in interdisciplinary meetings and other relevant meetings within CWPT and UHCW as required.
* Have broad knowledge of voluntary and independent organisations relevant to the specialist area, establishing effective relationships with local services to enhance care delivery and transfer.
* Attend staff meetings as required.
* Ensure all relevant information about a patient’s treatment or progress is communicated to other team members and agencies as required.
* Communicate reasoning for assessments and interventions to patients and carers, establishing full understanding and consent prior to delivery.
* Employ and utilise formal and non‑formal forms of assisted communication for patients or relatives with communication impairments.
* Communicate effectively with other team members, ensuring accurate understanding of delegated therapy programmes and safe individual practice alone in the community.
* Pass relevant communication from the team to Lead Therapists as required.
* Collaborate with key partnership organisations to ensure effective, safe delivery and transfer of care.
* Serve as a source of information for MDT members seeking advice or information.
* Keep relevant statistics and information as required by the department.


Qualifications and Requirements

* Provide an immunisation and vaccination report from a UK occupational health provider or UK GP.
* Hold a full UK driving licence (an international driving licence is not accepted).
