The Rotherham NHS Foundation Trust is an Integrated Acute and Community Trust which is committed to partnership working across Health and Care, built on a strong home first ethos. Working as part of a multi-disciplinary Health and Care team the role holder will be part of our multi-disciplinary Urgent Community Care team. They will provide high quality senior Nursing support for people requiring an urgent community response Out of Hours, enabling them to be cared for in their own home thereby reducing avoidable admissions and supporting timely discharge from the acute and community bed setting.
Main duties of the job
Provide effective triage and assessment on a rota basis ensuring Patients receive the right level of care, at the right time and in the right place according to the needs of the individual and within available health and social care capacity.Provide skilled Nursing advice, support and care to Patients, their family/supporters and carers.Have continuing responsibility for overall management of a Patients and caseloads.Provide strong clinical leadership to the urgent Out of Hours Nursing team.Promote good team working and an environment to enhance job satisfaction and ensure team members feel valued and empowered.
