You will be joining an HCD team of twelve RGN and RMN nurses covering the whole of England and Wales.
Mileage paid at 48p per mile (first 10,000 miles), then 25p per mile

About the Role
We are looking for a compassionate and proactive Registered Mental Health Nurse to support individuals with complex autism, and associated mental health issues, being discharged from secure units in the Lancaster and Yorkshire and Northern regions. Other RMN nurses will cover the Midlands and South.
This is a community-based support management role. You will play a key role in helping clients live independently and direct their own care, while also contributing to the development of a brand-new specialist department.
Key Responsibilities
Support clients and their families to manage care independently
Visit clients in their homes or secure units to assess care and support needs and develop care plans
Collaborate with internal Head Office teams to ensure seamless care delivery
Complete care and support plans, risk assessments, and documentation to a professional standard
Deliver training and conduct clinical observations for personal assistants
Undertake supervisions and annual reviews in line with CQC standards
Ensure ongoing monitoring and quality assurance of care provided
Identify and respond to any issues of non-compliance or risk
What Were Looking For
NMC-registered Mental Health Nurse (Band 5 or higher)
Experience supporting individuals with complex autism and associated mental health issues
Ability to work independently in the community, often as a lone worker
Skilled in care planning and risk assessment
Capable of delivering training within professional competencies
Competent in Microsoft Office and online care systems
Full UK driving licence and access to your own vehicle
Flexible and professional with a high attention to detail
