Job Summary

Our heart failure specialist nurse team play a pivotal and dynamic role in the care of our patients with heart failure. We have developed excellent systems of care, for example automated NTproBNP referrals and a heart failure clinical pathway, and are integrated with an excellent multi-disciplinary team including the community and palliative care teams. You will be supporting the team in the running of virtual ward for heart failure patients. You will be providing care in both inpatient and outpatient setting, from first diagnosis to end stage heart failure. Your role will be focused on ensuring medicines are optimised, providing lifestyle and self‑care advice to patients and carers and ensuring the patient has a comprehensive management plan on discharge. You will be preparing patient information for medicine and device MDTs and inputting data into the national heart failure database (NICOR).

You will receive a comprehensive induction programme, will be supported by senior HFSN and follow the BSH competency framework to guide development in your role. This role allows for flexible working as long as this meets the needs of the service.

Main Duties

* Work for the Trust as an autonomous specialist supporting the provision of excellent specialist care to patients with heart failure assisting in the diagnosis, treatment and follow up.
* Undertake comprehensive clinical assessment of heart failure patients.
* Work as an expert practitioner delivering evidence‑based care adhering to guidelines for heart failure management.
* Achieve heart failure competencies and keep skills and knowledge up to date with current research and clinical guidelines.
* Be responsible for recommending medications within scope of practice in line with independent non‑medical prescribing guidelines or be willing to work towards gaining these competencies.
* Liaise with key members of the MDT.
* Liaise with and refer patients to community services and other relevant inpatient services, primary care teams and cardiology follow up as required to ensure that patients receive seamless care along an integrated pathway.
* Provide specialist training to appropriate areas in the management and clinical pathways for heart failure patients.
* Develop the specialist nursing service based on patient experience feedback, audit, and research.
* Participate in the National Heart audit and assist with data input.
* Attend Multi‑disciplinary team meetings, acting as patient advocate and contribute to clinical decision making.
* Work flexibly to meet the needs of the service.
* Act as an effective role model and mentor promoting excellence through practice.


Person Specification

* Demonstrable ability to meet the Trust Values.


Education & Professional Qualifications

* First level registration.
* First level degree.
* Evidence of post registration qualification in Coronary/Cardiac care.
* Heart Failure Module.
* Physical Examination course and evidence of using these skills in practice.
* Educated to Master's degree or willing to work towards.
* Teaching/Mentorship/Assessment qualification.
* Non‑medical Independent Prescriber qualification or willing to work towards.


Desirable

* End of Life/Advanced communication qualification or willing to work towards.


Experience

* Minimum 5 years recent cardiology experience with minimum 2 years at band 6 in cardiac environment.
* Evidence of teaching and developing others.
* Ability to analyse and interpret ECG's, Echos and routine blood tests.
* Extensive experience of nursing patients living with heart failure.
* Experience of service planning/managing change e.g. provide examples of projects/research undertaken under own initiative.
* Experience of running nurse led clinics.
* Experience of managing own caseload of patients.
* Experience of the provision of cardiac rehabilitation and secondary prevention.
