Job Summary This role would suit either a newly qualified GP looking to develop their skills within a forward‑thinking and supportive practice or an experienced GP seeking to join a friendly multidisciplinary team. The position is offered on a fixed‑term salaried contract with the possibility of extension.
Responsibilities
Manage patients with acute and chronic conditions, providing preventive health services, medication reviews and personalised care planning.
Triage e‑consults in the hub alongside another GP.
Work collaboratively with a multidisciplinary team of GPs, nurses, pharmacists and allied health professionals to deliver patient‑centred care.
Respond to clinical queries, manage repeat prescriptions, review test results and prepare referral letters and other correspondence accurately.
Initiate investigations, review outcomes and refer patients to secondary care or other services where appropriate, ensuring treatment plans align with NICE guidance and chronic disease management protocols.
Maintain accurate clinical records and contribute to high standards of clinical governance and service delivery.
Collaborate with health and social care professionals to support integrated patient care.
Perform routine and emergency home visits as required, primarily covered by the paramedic and nurse practitioner team.
Work a six‑session‑per‑week schedule across three full days, with regular clinics and triage sessions as specified by the practice.
Qualifications
Qualification and Registration
GMC registration with a licence to practice in the UK.
MRCGP qualification or equivalent.
Newly qualified GP or experienced GP.
Clinical Expertise
Evidence‑based clinical care for acute and chronic conditions.
Proficiency in comprehensive patient assessments and care planning.
Independently prescribing within scope of practice and national/local guidelines.
Experience in preventive care and supporting patient self‑management.
