Job Overview
This is an exciting opportunity to join the Health Visiting Team in Monmouthshire as a Health Visitor. The post holder will manage a caseload within a defined population, aiming to increase family resilience, improve population health outcomes and reduce health inequalities, while safeguarding the wellbeing of children.
The post holder must register with the Nursing & Midwifery Council and be a registered SCPHN (Health Visiting); compliance with NMC standards and guidance is required.
Key Responsibilities
* Deliver the Universal Healthy Child Wales Programme to families with children under 5 years old, including evidence‑based interventions such as infant feeding, safer sleeping, perinatal mental health, and immunisations.
* Conduct screening programmes, for example, Newborn Blood Spot.
* Execute safeguarding interventions and apply the Family Resilience Assessment Tool (FRAIT) as scheduled.
* Implement targeted interventions for families with low resilience, provide intensive support.
* Initiate safeguarding procedures, work with families and carers to implement child protection plans.
* Participate in statutory safeguarding functions such as Court and MARAC/MAPPA processes.
* Apply risk management processes including lone working and risk assessments.
* Use the All Wales Health Visiting Acuity tool to ensure safe staffing levels.
* Promote new opportunities for integrated working and contribute to local partnership plans.
* Develop and maintain health profiles of an identified population to identify needs and influence service plans.
* Share responsibility for chairing Health Visiting Forums and team meetings.
* Contribute to the development of local policies and guidelines.

* Current NMC Registration.

Essential Qualifications
* SCPHN Health Visitor Post Graduate Diploma or Certificate.
* Nurse Prescribing.

* SCPHN Degree.
