The successful candidate will play a crucial role in the ongoingdevelopment and delivery of high-quality, patient-centred care for our growing patientpopulation. The post is for 30 hours per week over 4 days

Main duties of the job
Clinical Leadership
* Provide effective clinical leadership tothe nursing team.
* Champion evidence-based nursing care and best practices.
* Lead on clinical governance, quality assurance, and safety initiatives.
Patient Care & Chronic Disease Management
* Oversee chronic disease clinics andlong-term condition management.
* Deliver high-quality care across treatment planning, implementation, andreview.
* Promote proactive health education and disease prevention strategies.
* Deliver own nurse-led clinics (e.g., asthma, diabetes, hypertension).
* Supervise, mentor, and appraise nursingstaff and students.

Policy, Compliance & Clinical Standards
* Develop and review nursing protocols andclinical procedures. Ensure compliance with CQC regulations and NHS standards. Participate in audits, QI initiatives, and root cause analyses for incidents.
* Work closely with GPs, the PracticeManager, and multidisciplinary colleagues.
* Liaise with external agencies, community services, and safeguarding teams.
* Represent nursing in practice meetings and strategic planning.

* Immunisations: Routine, childhood, travel, and seasonal
* Cervical cytology and womens health services. -
* Chronic disease management including diabetes, asthma, COPD
* ECGs,
* Venepuncture,
* NHS Health Checks and opportunistic health promotion
* Safeguarding children and adults;
* Mental health assessments
* Referral coordination Infection prevention and control (IPC)
* Support with audit cycles and service evaluation
* Ensure proper use of SNOMED CT coding and chronic disease registers
* Uphold best practice in medication management and PGDs Encourage team learning and contribute to the practice's continuous improvement

Qualifications
* Registered Nurse (NMC).
