The Integrated Neighbourhood Teams are looking forexperienced District Nurses to join our community nursing team across theneighbourhood of Ashford & Stanwell.
The Community Teamvisit patients in their own home as well as care homes. We offer excellentopportunities for training and development of both your clinical and leadershipskills. The team has experienced Clinical Leads who will ensure that you havethe necessary support for your personal development. The community nursingservice works over a 7-day week to offering a variable shiftpattern and includes weekend and bank holiday working.
This post requires aspecialist District Nursing Degree, following your adult nursing qualification.NHS work and Community Nursing experience is essential.
You will besupported with a comprehensive induction and training package as needed, tohelp you settle in, with the time and opportunity to get to know the people onyour caseload, along with development opportunities.

Purpose
To ensure thedelivery of high quality, safe and clinical effective care for the CommunityNursing teams across a defined neighbourhood specialising in carecoordination. Achieves national andlocal policy and guidelines, meets regulations (CQC/Monitor) and professionalbody (NMC) requirements.
Principal duties to include:
Communication
Effective communication with Primary Care (GPs,practice nurses), social services, acute / community hospital staff, boroughcouncil and local voluntary sector
Utilises a range of effective communicationskills, tools and techniques that meet the needs of the patients and carers andis able to effectively overcome any barriers to communication and understanding.
Takes appropriate action on potential complex and/ or conflicting perspectives of choice of care options between staff,patients, carers and other disciplines or agencies
Actively participates in relevant professionaland organisational networks and contributes to meetings and working groups
Ensures daily team hand-over is effective withinacross a defined locality so that changes in patient condition are discussedand junior staff / students are supported
Communicates effectively with other professionalsacross agencies / disciplines where information sharing agreements are in placeand as agreed by the patient
Chairs and facilitates team meetings to ensureeffective cascade of information
Ensures contemporary and accurate record keepingwithin the locality whilst adhering to confidentiality and informationgovernance

Patient Care
Responsible for gathering, interpreting andacting upon complex information
Able to triage appropriately within referralcentres and during handover / MDTs toensure that nursing care is delivered in line with identified clinical need
Acts as a patient advocate and works to achievethe best outcomes for patients at all times.
Undertakes and documents holistic nursingassessments of patients, including those with multiple pathology and complexhealth and social needs with a plan of care agreed with the patient
Recognises own limitations in the provision ofclinical care and refers on to others accordingly
Provides skilled and effective nursing care in acompassionate, caring and professional manner,
Required to undertake physical examination of thecirculatory systems, undertaking Doppler assessments, venepuncture,catheterisation, rectal examination etc., and to use concentration whenundertaking calculations for administration of medication / supervisingadministration of medication
Personal and people development
Oversees induction programmes and orientation fornew staff and students aligned to the local District Nursing standards andservice principles
Maintains an overall view of competencies fornursing staff across defined DN teams and takes an active role in peer review,observing and signing off competencies for nurses
Responsible for ensuring clinical supervision,peer reviews and appraisals have been accessed across a defined locality
Provides clinical training and education andparticipates in the development of work based learning across the DistrictNursing service
Maintains effective learning environment in linewith University guidelines and NMC requirements
Acts as a role model demonstrating a level ofknowledge, competence and the highest professional and personal standard inwhich the District Nurse is required to emulate.
Specialist resource for palliative care acrossthe locality
Service Development
Manages change and implements strategy and newdevelopments across defined DN teams especially regarding palliative care
To be responsible and accountable for all aspectsof performance within the community nursing teams in regard to delivery toservice users
Works collaboratively with community nursecolleagues to ensure equity of service provision, promotion of good practiceand the sharing of resources
Supports the development and implementation oftechnology and new ways of working within teams
Contributes to the future development of DistrictNursing by participating in working groups, committees and planning groups asrequired.
Actively participates, advocates and help designlocal improvement initiatives that drive forward leadership and changestrategies within the service Service Improvement Facilitator Projects,Community Nursing Innovation Programme, local leadership initiatives.

Quality
To work alongside Clinical Leads, Team Leadersand District Nurses to monitor the quality of nursing service delivered.

Responsible for monitoring and promotingeffective and efficient use of resources such as equipment from the communityequipment store, dressings and nurse prescribed medication.

Disseminates research findings and theapplication / relevance to clinical practice
