Person specification

Experience

Essential criteria

1. Understanding of complex discharge planning in care of the elderly, stroke and vulnerable adults.
2. Experience of supporting patients’ specialised care needs to enable informed decision making

Desirable criteria

3. Previous experience of working with patients requiring CHC and Social care involvement
4. Experience of working in a hospital setting or relevant community service
5. Experience of working within an MDT

Technical Skills Competencies

Essential criteria

6. Assessment, planning and organisational skills
7. Ability to progress plans for discharge with internal / external agencies and family members
8. Use of IT including Windows, Word, Excel, Powerpoint,

Desirable criteria

9. Use of clinical IT systems

Qualifications

Essential criteria

10. NVQ level 3: health / social care management or equivalent

Personal Attributes

Essential criteria

11. Team working qualities including understanding the needs of other professions.
12. Self-motivated
13. Good time management
14. Ability to work under pressure
15. Able to prioritise own caseload
16. Flexibility to respond to urgent requests, changes to plans etc.

Knowledge

Essential criteria

17. Knowledge of complex discharge planning process.
18. Understanding of assessing and problem solving issues related to discharge planning
19. Understanding of NSF’s, CHC Framework, Delayed Discharged Act and Reimbursement.
20. Understanding of multidisciplinary assessment and treatment for older people / vulnerable adults within acute care and community settings.
