An exciting opportunity has arisen for a Social Worker to within the Mental Health Liaison Team (MHLT). MHLT are based within the Royal Stoke Hospital and see all patients who are referred to the service from A&E, emergency portals, inpatient wards and the wards at the Haywood Hospital.
This developmental post will be offered at a Band 5, with progression to Band 6 upon successful completion of the Assessed and Supported Year in Employment (ASYE). You will support and supervision by one of our Senior Social Workers.
As a Social Worker you will work within a multidisciplinary, providing assessment, signposting and safety planning to those referred to the service.
Main duties of the job
The post holder will be accountable on a day-to-day basis to the Team Leader, who will allocate, monitor and review all work through regular informal and formal supervision. The post holder will also be professionally supported by an experienced Social Worker.
* The post holder will have a key role in supporting assessments and a range of interventions for service users under the care of the team. The role will involve assessing, accessing, and implementing a range of interventions and providing guidance/advice and information for service users with acute or increasingly complex long-term conditions that could include mental health needs, learning difficulties or physical health difficulties.
* The post holder will be required to be an integrated and supportive member of a team, planning, actioning, monitoring and evaluating work with the supervision of an experienced Social Worker and the Team Leader.
* The post holder will be responsible for supporting the delivery of assessment and intervention for service users within the directorate and where relevant hospital/inpatient settings providing social work knowledge and expertise. The post holder will support requests for information, advice, guidance, referrals, initial assessments and interventions.
* The post holder is responsible for working collaboratively with other Trust services to provide seamless interventions for service users and carers.

Job responsibilities
* To carry out a range of assessments including holistic assessment with support from an experienced social worker or team leader.
* To provide social work support to individual adults/older adults/children and young people, their carers and families and involve them in all planning and actions taken, helping to signpost/refer (where appropriate) and/or develop motivation and daily living skills.
* To undertake Duty role with support from an experienced social worker or experienced member of the team.
* To communicate assessment information, care and support plans and risk factors to service users, carers, professionals, Emergency Services, MDT and Courts, as appropriate and in line with Information Governance. Communicate sensitive issues in an appropriate manner and address any barriers to participation and understanding.
* To communicate with a wide range of people including adults/older adults/children and young people, and carers/families, members of multi-disciplinary teams, and staff in external agencies including Primary and Secondary Health Care/Mental Health services using a variety of methods including Information and Computer Technology.
* To coordinate discharge with individuals in collaboration with hospital/in-patient services, be part of proportionate involvement in the delivery of services post discharge where appropriate.
* To uphold the principles of wellbeing and ensure people can access their rights and entitlements including access to social care resources under a range of relevant statute and eligibility criteria including (but not limited to) the Care Act (2014), The Mental Health Act (1983/2007), the Mental Capacity Act (2005) and the Human Rights Act (1998).
Person Specification
* Professional Social Work qualification, DipSW, CQSW, CSS, BA, MA or equivalent.
* Current registration with the Professional Social Work regulatory body at all times.
* Experience of working with Adults and/or older Adults and/or Children and Young people with mental health needs or learning disability.
* Discovery and Barring Service Check: This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
