Overview
We are seeking a dedicated and compassionate Band 4 Rehabilitation Assistant Practitioner to join the Community Independence Service (CIS) in Hammersmith & Fulham, a collaborative initiative between West London NHS Trust and CNWL Foundation Trust. The CIS delivers planned and unplanned nursing and therapy interventions to older residents experiencing recent physical health deterioration, with the aim of promoting recovery, independence, and preventing hospital admissions. The role involves delivering care in patients’ homes or designated community settings, assessing and implementing treatment plans for a delegated caseload using clinical reasoning and analytical skills. The post includes managing the ordering and tracking of equipment and supplies, ensuring timely delivery and functionality to support effective patient care. The role offers the opportunity to be part of a committed multidisciplinary team making a tangible difference in the lives of older adults across Hammersmith & Fulham and the broader community rehabilitation network in Westminster and the Royal Borough of Kensington & Chelsea.

Responsibilities
* Independently assess a defined caseload of patients using clinical reasoning to identify rehabilitation needs and establish personalised goals; accurately document all interventions.
* Deliver nursing, physiotherapy, and occupational therapy treatments as part of agreed care plans, across settings including patients’ homes; work with families, carers, and support workers.
* Provide both individual and group therapy and support complex cases under supervision of qualified professionals; escalate concerns beyond the scope of practice when appropriate.
* Order, fit and train patients and carers in mobility aids and equipment; manage equipment orders and ensure functionality.
* Support the development of therapy students and new staff.
* Maintain effective communication, including with patients who have neurological impairments; maintain accurate records in line with Trust standards.
* Contribute to multidisciplinary team discussions; manage own caseload; assist in service development and audits; follow consent procedures and apply outcome measures.
* Carry out risk assessments to support safe and effective rehabilitation interventions.
Qualifications and Skills
* Band 4 Rehabilitation Assistant Practitioner or equivalent; ability to work across nursing, physiotherapy, and occupational therapy disciplines.
* Clinical reasoning to identify rehabilitation needs and establish personalised goals.
* Ability to work across a range of settings including patients’ homes; effective collaboration with families, carers and support workers.
* Experience in delivering both individual and group therapy; ability to escalate concerns as needed.
* Experience with ordering and fitting mobility aids and equipment; training patients and carers in their use.
* Strong communication skills; experience with patients who have neurological impairments.
* Ability to document accurately and maintain records in line with Trust standards; contribute to MDT discussions, audits and service development.
