Overview
Specialist Community Public Health Nurse Triage (SCPHN) role at Buy A Boat Now. The post holder will be a senior member of staff working as a triage practitioner within the integrated 0 to 19 Partnership. They will provide the expertise, knowledge, resource and support to the team managing enquiries from pregnant women, families, carers, children and young people in Torbay.
The post holder will provide a comprehensive triage service (for the public and professionals), access and update the Electronic Health Record to ensure continuity of care and to make appropriate recommendations and interventions. They will work with the 0 to 19 Partnership workforces to ensure pathways are followed in supporting children, young people, families and carers. The service includes the identification of vulnerable children, including those whose welfare and safety may be at risk, working within child protection procedures and in collaboration with other agencies. The post holder will have the opportunity as part of a team to shape the ongoing development of the role within the Universal Team allocation process and be the Advice line first point of contact for families contacting the Admin Hub. The post is office based.

Responsibilities
* Provide a comprehensive triage service for the public and professionals and manage enquiries from pregnant women, families, carers, children and young people.
* Access and update the Electronic Health Record to ensure continuity of care and make appropriate recommendations and interventions.
* Collaborate with the 0 to 19 Partnership workforces to ensure pathways are followed in supporting children, young people, families and carers.
* Identify vulnerable children at risk, work within child protection procedures, and coordinate with other agencies as required.
* Contribute as part of a team to shape the ongoing development of the SCPHN role within the Universal Team allocation process and serve as the Advice line first point of contact for families contacting the Admin Hub.
* Work within an office-based setting as part of the SCPHN team.

Qualifications & Requirements
* Qualified Specialist Community Public Health Nurse (SCPHN) with relevant experience in triage, public health nursing or health visiting settings.
* Experience with triage, safeguarding, and working with diverse families and young people.
* Ability to work collaboratively with multi-agency teams and professionals.
