Overview
To be responsible for assessing, triaging, planning, implementing and evaluating care which may be complex, sensitive and unpredictable, with the support of a senior nurse. To support the coordination and delivery of care, applying clinical judgment in day-to-day changes in patients\' needs and circumstances external to the individual, determining when to seek advice from senior clinical or management staff. This includes response to immediate crisis. To maintain accurate records which are confidential, contemporaneous, legible and reflect all care given. Records may be paper or IT based and must be maintained as specified in the Trust policies, professional body guidelines and government directives.
To provide clinical supervision to other staff and students; promote and support people in their care and treatment to help them manage their own health and wellbeing, maximise independence and facilitate health promotion. Depending on role, work within a defined geographical area undertaking specific clinical nursing duties to patients in settings such as their own homes, care homes, other community locations or clinic settings.
To work within the scope of the NMC professional code of practice and, where needed, be prepared to move across the organisation (e.g., in a pandemic). To work within Livewell Southwest policies and to share knowledge and empower others. Some services may require additional training to deliver intravenous medication or additional specialist nursing skills. The role may require co-ordination of caseloads and collaboration with managers and service/practice leads. Provide holistic evidence-based / best practice care to patients in accordance with national and organisational policies, procedures and individual care plans.
To act as a clinical advisor on health promotions within areas of expertise and to act as a Link Nurse. Undertake required training to develop competencies for the role. If relevant, to be or work towards a nurse prescriber qualification (NMC V150). The role may involve 7 days of working, including bank holidays, as set out in the contract of employment. In the absence of the Clinical Nurse Specialist / Caseload Holder, be responsible for managing own workload and delegating nursing care to appropriately skilled staff with appropriate support.
To assume responsibility as delegated by a senior member of staff; to act as a role model for all staff and students, demonstrating specialist clinical skills and high standards of practice and professional conduct. To share knowledge and information to promote a cohesive team; to take responsibility for personal and professional development, maintaining competence, knowledge and skills commensurate with the role.
To assess, teach, support and supervise colleagues including pre-registration and medical students. To participate in team activities to develop a cohesive and supportive team with openness. To facilitate and participate in the development of team members, monitoring sickness, line management, appraisals, mandatory training, competencies, clinical supervision and staff performance as directed by the Service Lead. To participate in the recruitment and selection of new staff.
To participate in coordinator/duty rota as required.
