See documents enclosed Currentlythe medical team consists of two Consultants in Palliative Medicine and one Specialty Doctor who work closely together to provide seamless clinical care and leadership to all facets of Specialist Palliative and End of Life Care within Bolton. They Post holder will deliver clinical interventions across care settings providing advice,education and support to patients, those important to them, and other health care professionals. They will manage complex symptoms, support the prevention of inappropriate admissions to hospital and promote Advance Care Planning. In addition, they will be able to contribute to the development of services, teaching and research.
Active participation in audit is expected and is supported by an active Clinical Effectiveness Department.
