Sister/Charge Nurse / Clinical Practitioner: Care Navigation Centre
Care Navigation Centre is a well-established team in Walsall providing a single point of access for professionals seeking to keep patients in their own home or to avoid an ED admission.

Main duties of the job
We are seeking a registered practitioner (nurse / paramedic) with evidence of post-registration practice at Band 5 to join our Care Navigation Centre. The role involves triaging and referring patients to appropriate community services, working flexibly to meet service demands, and establishing strong links with community and acute colleagues. Good IT skills and a pleasant telephone manner are essential. Experience in multidisciplinary and multi-agency working is required.
Service provision operates 24/7 on a pro-rata rota basis.

Salary: £38,682 to £46,580 per annum, pro-rata.
