Critical Care Outreach Sister/Charge Nurse (XR07)

Anexciting opportunity has arisen to join the Critical Care Outreach Team as aSenior Sister / Charge Nurse. The post holder will be experienced in thefield of Critical Care, experience working as part of the Critical CareOutreach Team is desirable.

Thesuccessful candidate will be allocated to take charge of shifts, where theywill coordinate the safe running of the team and workload. This includescross site working, lone working and providing ongoing support and leadershipto junior Critical Care Outreach team members.

Thesuccessful candidate will provide advanced clinical assessment of thedeteriorating adult patient in a variety of clinical areas throughout theTrust, deliver education to ward-based teams on the recognition andmanagement of the deteriorating adult patient and support the Deputy Matronand Deteriorating Patient Lead to implement Trust wide quality improvementinitiatives.


Main duties of the job

Therole provides extensive clinical advice, education and support to ward basedmulti-disciplinary teams across a variety of clinical areas throughout thetrust.

Thepost holder will work as an accountable, autonomous practitioner to providecomprehensive advanced assessment and care of the deteriorating/acutely illadult patient leading to early intervention and treatment.

Thepost holder will work in partnership with the Allied Healthcare ProfessionalsTeam to initiate appropriate care to prevent further physiologicaldeterioration, identifying and referring those patients who require admissionto critical care.

Thepost holder will act as a clinical role model, promoting qualityevidence-based practice. Multi-disciplinary team liaison, education andtraining in the care and management of the deteriorating/acutely ill adultwill be key responsibilities.

JOBPURPOSE

Qualifications

RegisteredNurse (Level 1).

CriticalCare Course or equivalent competencies

Recognisedteaching/assessing qualification.

NationalOutreach Framework Enhanced Competencies.

Educatedto post graduate degree level or equivalent experience.

AdvancedLife Support provider.

Experience

SignificantCritical Care Outreach experience at sister/charge nurse level.

Workingwith a wide range of professionals including; medical, nursing and managementcolleagues.

Clinical

Deliverand maintain a high standard of patient focused care in accordance with LeedsTeaching Hospitals Trust guidelines and policies.

Workwithin frequently challenging clinical environments with high levels ofphysical and mental effort.

Haveknowledge of pathophysiology, anatomy and physiology in order to assess,plan, implement and evaluate treatment of the critically ill patient. Able tomake expert clinical judgements involving complex situations which requiresanalysis and interpretation to decide on the best course of action/treatment.

Competentlyand confidently care for the deteriorating/acutely ill adult utilising expertcritical care skills and knowledge in a variety of clinical settings withindirect supervision as part of the multi-disciplinary team

Providerapid response and support for patients identified by the National EarlyWarning Score.

Undertakea comprehensive advanced patient assessment based on history taking andclinical examination and implement a management/treatment plan.

Providedirect patient care and support at the bedside until the patient isstabilised or transferred to an area with a higher level of care. Initiatetherapeutic interventions for example to manage haemodynamic and respiratoryinstability.

Referto other members of the MDT and other specialities as appropriate.

Participatein the stabilisation and transfer of critically ill patients to anappropriate environment.

Competentin advanced clinical skills such as venepuncture, cannulation, arterialstabs, tracheostomy decannulation, airway management and cardiopulmonaryresuscitation.

Accuratelyrequest pathology and radiological investigations and make clinical decisionsbased upon the results.

Utilisehighly developed communication and negotiation skills to deal withchallenging/conflict situations ensuring patient centred care.

Workautonomously taking responsibility for own decision making, referring to theappropriate physician/ICU Consultant/Outreach Team Leader when advice andfurther clinical input is needed or when care and treatment goes beyondnormal service parameters or scope of professional practice.

Carrythe crash bleep and respond to 2222 calls.

Whererequired, carry the critical care bed managers bleep out of hours, liaisingwith the sisters/charge nurses to co-ordinate flow throughout critical care.

Respectpatients rights to participate in decisions about their care and be able toadvise them on complex and personal matters.

Communicatehighly sensitive information to patients and their relatives, showing empathyand providing psychological support.

Maintainup to date clinical records in accordance with the most up-to-date NMC andTrust guidance.

Actas a clinical role model, which reflects the philosophy, beliefs and clinicalstandards of the outreach service.

Maintaina safe working environment.

Demonstratean understanding of key infection prevention and control policies including,hand hygiene, MRSA, transfer of patients.
