The post holder, as an employee of Rochdale Health Alliance, will work within the Rochdale North Primary Care Network Clinical Pharmacist service for GP practices as part of a multidisciplinary team.

Main duties of the job
Providing expertise in clinical medicines management through face-to-face structured medication reviews, management of long-term conditions, transfer of care, and systems for safer prescribing. Manage repeat prescription authorizations and reauthorizations.
The post holder will provide leadership on quality improvement and clinical audit around medications, contributing to the Quality and Outcomes Framework and local contract performance indicators.
The postholder will undertake the CPPE pathway (or have already completed this).

Rochdale Health Alliance (RHA), established in 2016 by GP practices across Rochdale Borough, aims to improve care and health outcomes for patients, sustain local primary care practices, and lead innovative ways of working to keep GP services central to the integrated system.
We have 31 member practices across Heywood, Middleton, and Rochdale, supported by a dedicated team with expertise in finance, HR, communications, project management, and strategy, sharing knowledge to enhance resources.
Job responsibilities
The duties may include, but are not limited to:
1. Long-term conditions: Supporting patients with conditions like respiratory, cardiovascular, and diabetes, optimizing medication use, and making recommendations for improvements.
2. Patient-facing clinical medication reviews: Conducting reviews and providing recommendations to the healthcare team.
3. Care home medication reviews: Similar reviews with additional focus on safety in care homes.
4. Domiciliary medication reviews: Conducting reviews for homebound patients and participating in multidisciplinary case conferences.
5. Identification of high-risk patients: Using computer searches to identify patients at risk from medications.
6. Unplanned hospital admissions: Reviewing medication use linked to hospital admissions and implementing reduction strategies.
7. Management of minor ailments: Managing common conditions within scope and referring as appropriate.
8. Patient medicines support: Providing clinics and telephone support for patient queries.
9. Medicines at discharge: Reconciling medicines post-hospital discharge and ensuring continuity.
10. Medicine information: Responding to inquiries from staff and patients, providing solutions, and follow-up.
11. Signposting: Referring patients to appropriate care levels.
12. Repeat prescribing: Managing policies and processes for repeat prescriptions, reviews, and monitoring.
13. Data analysis and audits: Interpreting medicines data, conducting audits, and implementing changes.
14. Guideline implementation: Monitoring prescribing against guidelines and maintaining formulary compliance.
15. Education and training: Providing therapeutic and medicines optimization education.
16. Regulatory compliance: Ensuring CQC standards are met regarding medicines.
17. Public health: Supporting campaigns and programs.
18. Collaborative working: Engaging with multidisciplinary teams, stakeholders, and external agencies to improve patient care.
Person Specification
Experience
* Commitment to reducing health inequalities and effective communication with stakeholders.
* Ability to recognize personal limitations and refer appropriately.
* Experience with chronic and acute conditions, partnership working, and understanding social determinants of health.
* Knowledge of IT systems, medicines optimization, risk management, research governance, and collaborative practice.
* Minimum two years' experience as a pharmacist, with demonstrated clinical knowledge and evidence-based healthcare understanding.
Qualifications
* Degree in pharmacy and GPhC registration.
* Clinical diploma.
* Independent prescribing qualification.
