An exciting opportunity has risen for enthusiastic and motivated individuals to join our Reactive Emergency Assessment Community Team (REACT) as a Registered Nurse. The REACT team is a multi‑disciplinary integrated community service within ESNEFT incorporating OPAT (Outpatient Parenteral Antibiotics Therapy), OOHs (Out of Hours) and Virtual Ward pathways patients.
Our team delivers excellent patient focused care to patients in the East Suffolk community. Could you be a valued key member of our team? We will offer an individually tailored induction, training and support for the successful candidate.
The role involves travelling throughout East Suffolk delivering acute care in patients' home as a part of the REACT nursing team. The post holder will be working in an internal rotation. The successful candidate will be expected to work early shifts, late and night shifts including weekends and bank holidays. During out of hours between 18:00‑8:00 the Nurse will be accompanied with a Health Care Assistant. Pool cars are available for staff use; however there may be occasions where you required to use your own vehicle for work purpose. In such circumstances, business mileage can be claimed in line with personal vehicle use policies. It is essential that the post holder holds a valid driving licence and has appropriate business cover for the use of their personal vehicle for work related travel.

Main duties of the job
* To assess, plan and deliver healthcare for patients, for example with long term conditions, palliative care and rehabilitation needs, to achieve quality of life and independence where possible.
* To work within the integrated team to facilitate early discharge from hospital.
* To work within the integrated team to prevent unnecessary admission to hospital.
* To work with all health care professionals, and statutory/non‑statutory agencies to provide a seamless, integrated service to our service users.
* Triage, assess, plan, implement and evaluate all aspects of patient care, and develop care plans which may need to be delivered from a range of options.
* Ensure informed consent is obtained and documented prior to initiating interventions.
* Following a holistic assessment, devise an individualised, evidence‑based plan of care for each patient, modifying it as required.

Qualifications
* RN Qualification (Part 1 of register) or equivalent
* Current NMC registration
* IV Administration / cannulation
* ENB 998/mentorship preparation or equivalent
* Mentorship qualification is essential for the post, but not essential at interview
* Previous experience working in the community
Experience
* Proven ability to communicate effectively using the English language including where there are barriers to understanding
* Providing high standards of care & use own initiative
* Demonstrated ability to manage stressful situations
* Ability to undertake patient care and input accurate data/patient records
* Experience working in a team
* Able to manoeuvre patients using handling aids
* Clinical reasoning skills
* IT and standard keyboard skills
