Job Overview

The post-holder works as an expert clinical practitioner and is responsible for the delivery of patient-focused programmes of care. The post holder will Proactively co-ordinate bed management over a 24hour period. To maximise the efficient and effective utilisation of the Trust's beds by co-ordinating with the support of the Lead Clinical Site Manager, ED and the wards to ensure all access targets are met both for ED and waiting lists. Provide an 'out of hours' site management service to include operational management and clinical lead for the hospital at night.

Main duties of the job

To act as management representative to all staff out of hours. Accountable for the management of the 'site' acting as a co-ordinator of services. To take the appropriate action relating to complaints, accidents and untoward incidents involving patients, staff and visitors. To ensure compliance with the Trust policies and procedures, Ensuring the hospital is adequately resourced with nursing staff out of hours by re-deploying staff to cover last minute sickness and minimising the use of NHSP. Assume direct responsibility for:-Communicating with outside agencies, To safeguard the interests of the Trust by reporting problems/issues relating to the hospital site where health and safety may be at risk. Direct contact for on call managers. Facilitate the admission of emergency patients within the major principles of the Trust and national policies and guidance. Establish an effective working relationship with ward nursing staff to ensure collection of accurate bed

information Liaise with ED to ensure beds are allocated to patients Establish daily the availability of critical care beds. Assist in the establishment of monitoring systems for bed capacity. Instigate the immediate response to Major Incident by setting up the incident room.

The Clinical Site Manager will fulfil their role through the following means:

Site Management

To act as management representative to all staff out of hours on behalf of line managers.
To assume accountability for the management of the 'site' acting as a co-ordinator of services.
To manage clinical and non-clinical emergency situations. The site manager should be the first port of call for all issues and remain responsible for any incident until it has been handed over to the appropriate manager or has been resolved.
To assume responsibility for mental Health issues i.e. accepting section papers on behalf of the Trust.
To take the appropriate action relating to complaints, accidents and untoward incidents involving patients, staff and visitors. To ensure compliance with the Trust policies and procedures, instigating the disciplinary procedure and suspending staff if necessary.
Ensuring the hospital is adequately resourced with nursing staff out of hours by re-deploying staff to cover last minute sickness and minimising the use of NHSP/Agency staff.
To act up as required for the Lead Site Manager.
Assume direct responsibility for:-

Communicating with outside agencies, e.g. TVEA, other trusts.

To safeguard the interests of the Trust by reporting problems/issues relating to the hospital site where health and safety may be at risk.

Direct contact for on call managers. All calls to the on call manager are directed through the Clinical Site Manger.

Works With The Bed Manager To

Facilitate the admission of elective and emergency patients within the major principles of the Trust and national policies and guidance
Establish an effective working relationship with ward nursing staff to ensure collection of accurate information with regard to bed availability, daily cancellations and outlying patients.
Liaise with the nurse in charge of ED to ensure beds are allocated to patients within the 4 hour target.
Attend daily operational meetings to communicate capacity issues i.e. number of available beds and predicted number of discharges etc.
Liaise closely with ward colleagues to ensure all measures are taken to provide beds for admission of emergency and elective patients in a timely manner to prevent ED breeches and cancer breeches.
Establish daily the availability of critical care beds in liaison with the nurse in charge of ICU
To instigate and implement the escalation policy as required.
Assist in the establishment of monitoring systems to highlight reasons for bed shortages.
Instigate the immediate response to Major Incident by setting up the incident room.

Clinical Practice: Out of Hours

As The Clinical Lead For The Hospital At Night Team The CSM Will Contribute To All Aspects Of Care Through The Following Interventions

Taking the lead and responsibility for co-ordinating patient care for the Hospital at Night team, delegating the workload via the ibleep system.
Respond to cardiac arrests out of hours as well as non clinical areas in hours
Undertake clinical skills as necessary:
Act as Fire Officer out of hours

Be a role model for professional practice clinical leadership and management.

Education and Training

Allocate work and assess performance of the team members objectively against set criteria/competencies. Be responsible for ensuring that individual Performance Review and Personal Development Plans are completed for each staff member. Provide clear feedback to team members in a way that is conducive to maintaining and improving performance. Work closely with the Human Resources Department as appropriate.
Demonstrate excellent personal communication. Establish and maintain effective two-way communication channels with individuals and groups. Demonstrate appropriate assertiveness and ability to challenge others when the rights of patients and others may be infringed.
Prioritise workload to meet targets. Ensure that quality and risk management issues are appropriately escalated.
Be responsible for ensuring the clinical area practices within Data Protection/Confidentiality/Caldicott principles.
Actively promote diversity, including race diversity, both in the delivery of the Directorate's services and in the recruitment, management and development of staff within the clinical area. Generate and sustain relationships that promote dignity, rights and responsibilities. Identify and take action to address discrimination and oppression.
Promote interdisciplinary working, with outcomes aimed at the best interest of patients.

Teaching, Training and Research

Promote/develop and maintain a culture within clinical environment which challenges and questions practice, and promotes the quality of thinking amongst practitioners.
Contribute to the implementation of the development of the service. Be aware of new evidence and research in both general and specialist fields. Support staff in the analysis of evidence. Work creatively to develop practice and new ways of working, taking the lead in the change management process when appropriate.

Role Responsibilities

Monitor and maintain the health, safety and security of self and others in work area. Identify and assess risks in work activities, and develop strategies to manage these effectively. Ensure team works within risk management/Occupational Health legislation and Trust procedures.
Develop own skills, taking responsibility for continuing professional development and performance, maintaining own portfolio in accordance with post-registration requirements. Work to achieve competencies appropriate to grade. Identify own development needs in relation to current practice and future plans, setting personal development objectives.
Act-up for Lead Clinical Site Manager as required.
