An exciting opportunity has arisen for the recruitment of a Care Co-ordinator within the Medicine Division. The post holder will be an integral member of the team responsible for managing and facilitating patient discharges from the medical wards.
The successful post holder will work closely with ward staff, divisional operational teams, patient flow and Discharge Planning within West Suffolk Foundation Trust as well as wider social services department's

Education and qualifications
* oAchievement through study to level 3 or above (NVQ, GCSE A-C, 1 st Diploma, Apprenticeship)
* oLevel 2 Maths & English qualification or equivalent
Experience and knowledge
* oAbility to work on own initiative and within a team Understands the importance of patient confidentiality
* oExperience of working in a healthcare setting
* oKnowledge of NHS Continuing Healthcare & Delayed Transfers of Care processes
* oExperience in using patient information systems PAS, eCare, Evolve LOSPT, Whiteboard, EPRO)
