Minimum Qualifications:
Degree: Master of Social Work (M.S.W.), accredited by CSWE.
Certification: Current, unrestricted clinical social work license in the U.S. or territories.
Experience: At least two years in care coordination, discharge planning, or case management in a medical setting within the last four years.
Required Skills/Abilities
Manage EFMP-M in accordance with DoD and military policies.
Use clinical assessment skills for timely identification of families with special medical and educational needs.
Collaborate with various service providers and ensure proper data management and reporting.
Coordinate enrollment, travel screening, and maintain documentation for families with special needs.
Participate in meetings, training, and support the integration of EFMP services.
Conduct quarterly case reviews and coordinate patient assessments and care plans.
Follow all applicable policies, complete documentation, and maintain training requirements.
Apply skills in assessment, planning, facilitation, and advocacy for families.
Possess knowledge of psychosocial practice, human development, and social systems.
Work effectively with service members and veterans facing complex issues.
Maintain confidentiality in accordance with HIPAA and accreditation standards.
Proficient in Microsoft Office, data management, and effective communication skills.
