Your job as a Hospital Discharge Co-ordinator will be to manage direct partnerships with Health Colleagues, ensuring discharges are planned in a timely and safe fashion. This role represents primarily the STEPS service and as such needs to convey a high level of professionalism and reliability and is also pivotal to supporting our health partners in times of crisis and escalation when discharges need to be expediated. At times of escalation or extreme pressure there may be requirement for this role to provide peripatetic cover to support the Community teams as required. Based within Treliske Hospital, or at times, local community hospitals, to ensure the role contributes to effectively across the acute and community hospitals for early identification for people who would benefit for reablement and support the successful delivery of STEPS contractual KPI’s and current contractual requirements for new “Starts” per week and supporting the offer of a TECH enabled care approach. The post holder will work closely with the Discharge Reablement co-ordinators, Community Care Supervisors and STEPs RN/Therapy Lead. In the community providing hands on support and supervision to both staff and customers and identifying and assessing that customers have the right level of care going forward. About the role As a rotational post, the role encompasses a variety of different elements which depending on operational needs and may be based in one or more of the options below: In the acute hospital settings as key links to identify patients ready for discharge and supporting health colleagues to facilitate safe discharges home. What you'll be doing Key responsibilities for this role holder will include: • Developing and nurturing strong partnership relationships with our Health partners. • Participating in the daily flow hub discussions. • Meeting Patients on the ward and discussing their needs, ensuring they are appropriate for either STEPS, or Care & Support and that they understand the principles behind Reablement or long-term home care. • Planning safe discharges, collating relevant information and preparing good quality care plans to support Community colleagues with the processing of new referrals. • Feedback to operational teams when identifying potential pressure points in the system are likely. • Participate in Daily Operational meetings as required to support discharge planning. • Plan and facilitate information and awareness sessions for Health Partners based at the acute hospital site. • Daily report for accepted referrals to relevant the RN/Therapy Lead, Registered Manager reablement Manager or local Deputy Managers. You will need: The following qualifications and experience are essential: Minimum level 2/3 qualification in Health and Social Care or equivalent. Experience in providing direct care in a work setting. Awareness of developments within Health and Social Care, multi-agency care issues coupled with relevant health care training. Ability to undertake refreshed Common Induction Standards relevant to the role, if not already undertaken. Knowledge and Skills: Understands and applies the principles of the Data Protection Act, Freedom of Information Act, Equality Act, Human Rights Act Putting People First agenda, CQC essential standards and the Management common induction standards and codes of practice. Understands and applies person centred customer care policies and practices in all day-to-day activities. Understands and applies multi- agency safeguarding policies for vulnerable adults. Displays empathy and patience when dealing with customers. Has outline knowledge of tools and techniques for dealing with challenging behaviours and coaches others in their use. Demonstrates good skills and abilities of Microsoft Office packages. Proficient in understanding of multi-agency approaches to person centred care and can identify key professionals whose involvement would be of benefit to the outcomes for the service user. Demonstrates sufficient knowledge and understanding of Health and Safety policy and practice and can undertake work effectively and conduct risk assessments. Ability to work on own initiative and under pressure. Full Valid driving licence. The duties of the role involve travel on a regular basis. It is a condition of employment that the role holder can exercise satisfactory travel mobility in order to fulfil the obligations of the role. For those journeys where an alternative form of transport is unavailable or impracticable the role holder will be required to provide a suitable vehicle. Our offer to you: Salary: £ 27,810 pro rata Working Hours: Part time – 22.5 hours per week (Monday to Wednesday) Contract Type: Permanent Base: Treliske Hospital, acute hospital or at times, local community hospitals.
