Overview
Occupational Therapist role at Manchester University NHS Foundation Trust (MFT). The post involves working within the discharge to assess team delivering holistic assessments and interventions to patients in their own homes or place of residence. Independent travel within the local geographical area to deliver the role is essential.
The Manchester community response services offer a seven day service.
Responsibilities
* Manage a clinical caseload of patients, including those with complex and highly complex needs.
* Supervise band 3 support staff with caseload management.
* Assess, plan, implement, evaluate and modify individualized treatment programmes in patients homes, working as a lone worker in the community.
* Collaborate with colleagues to ensure patient needs and goals are met, using approaches that promote choice, participation and quality of life.
* Triages and manages patients on appropriate and timely pathways of care by liaising with GPs, consultants, specialist equipment agencies, social services partners and voluntary agencies.
* Support patient flow by contributing to the timely discharge of patients from the acute setting.
* Provide complex professional and clinical advice to patients, family members, carers and health and social care professionals (including GPs, therapy colleagues and care managers).
Qualifications and Requirements
* Recognised Occupational Therapy qualification and current HCPC registration.
* Experience of working in the NHS and in community services.
* Ability to demonstrate appropriate clinical skills in occupational therapy and experience in administering person-centred assessments.
* Experience in implementing and evaluating occupational therapy interventions in a fast-paced environment.
