Job Description:
We are seeking a compassionate and experienced Care Manager to oversee our supported living and assisted living programs. The Care Manager will be responsible for coordinating care services, managing a team of care providers, and ensuring the well-being of residents in our supported living and assisted living facilities. This role requires strong leadership skills, a deep sense of empathy, and a commitment to providing person-centered care.
Key Responsibilities:
1. Develop and implement care plans tailored to the individual needs of residents
2. Supervise and support a team of care providers, ensuring the delivery of high-quality care services
3. Collaborate with residents, families, and healthcare professionals to create a supportive living environment
4. Monitor the well-being of residents, conduct assessments, and adjust care plans as necessary
5. Ensure compliance with regulatory standards and best practices in supported living and assisted living
6. Maintain a safe and welcoming environment for residents and staff
Qualifications:
1. Degree in Health & Social Care or a related field
2. Previous experience in a care management role, preferably in supported living or assisted living settings
3. Strong leadership and communication skills
4. Ability to work collaboratively with a multidisciplinary team
5. Knowledge of regulations and guidelines governing supported living and assisted living facilities
