We are seeking a skilled and motivated practitioner to join our Adult Social Care team. The successful candidate will be responsible for delivering high‑quality assessments, reviews, and planning under the Care Act, ensuring adults receive safe, effective, and person‑centred support.
Key Responsibilities
Complete Care Act eligibility assessments to identify needs and required outcomes.
Undertake care and support plan reviews, ensuring packages remain appropriate and cost‑effective.
Develop person‑centred care and support plans that promote wellbeing and independence.
Carry out safeguarding enquiries in line with Section 42 responsibilities.
Undertake Mental Capacity Act assessments and contribute to best‑interest decision-making.
Maintain accurate case records and work within statutory and local procedures.
Ideal Candidate
Previous experience using the Eclipse Care System is highly desirable.
Experience working within North Northamptonshire Council (NNC) or West Northamptonshire Council (WNC) would be a significant advantage.
Strong understanding of Care Act legislation, safeguarding, and MCA.
Confident managing a varied caseload in a busy environment.
