Social Worker Review and Extra Care Team
The closing date is 12 November 2025.
An exciting opportunity has arisen for an experienced Social Work Practitioner to join Salford Adult Social Care.
We strive to support people to live well and to be safe at home. We do this by having strength‑based conversations to promote wellbeing, working closely with other organisations to ensure that people are well connected with local communities and can move toward achieving personal outcomes.
The Adult Social Care workforce aims to improve outcomes for those living in Salford by developing a skilled and motivated team, investing in continued professional development and keeping social‑work practice of a high standard. You should hold a degree in social work, be registered with Social Work England and possess evidence of continuing professional development, alongside a strong understanding of the Care Act, Mental Capacity Act and Safeguarding Adults.
Salford is an exciting place to work and live within Greater Manchester’s fastest growing economy, home to the BBC, Granada and many digital and creative businesses.
Main duties of the job
The main duties include completing strengths‑based assessments, promoting wellbeing by utilising resources from social care, health and other voluntary or statutory agencies. The role involves undertaking initial assessments, proactively handling complex and legal issues, crisis planning, providing information, advice, signposting and transferring to other social work teams or services as required. You will identify needs for further assessment under section 9 and 10 of the Care Act, transfer these to the relevant teams, and complete support planning and reviews. The safeguarding function includes initial information gathering, risk assessment, safety planning and determining if Safeguarding Adult Enquiries under section 42 of the Care Act are required. The role demands resilient practitioners who can manage competing demands, prioritise effectively, and stay calm under pressure.
