Patient Pathway Co-ordinator

Hours of Work 37.5 hrs per week

An exciting opportunity has arisen within the Cancer Services Team for a full-time, permanent, 37.5 hours per week, Band 3 Patient Pathway Coordinator.


Main duties of the job

You will be required to work closely with clinicians, specialist nurses, secretaries and other members of the Multi-Disciplinary Teams (MDT) and associated departments, to ensure that patients on a cancer pathway are proactively managed. You will be required to coordinate the patient journey from referral, through diagnostics and treatment, highlighting any potential delays in the pathway promptly.

Candidates must have excellent organisational, communication, interpersonal skills and possess the ability to have difficult conversations in challenging environments.

Experience of working within a Healthcare setting is essential for this role alongside an in depth knowledge of National Cancer Waiting Times targets.

About us

The post holder is required to work closely with the Multi-Disciplinary Team (MDT) Coordinators, Clinicians, Specialist Nurses, Secretaries and all members of the MDT team within the body site and with associated departments, to ensure that patients on a cancer pathway are actively monitored through the patient pathway to provide a high quality, safe and compassionate healthcare service.

You will be required to track the patient journey from referral, diagnostics and treatment, highlighting any potential delays in the pathway and ensuring accurate data collection to support the process.

You will be required to highlight relevant patients to the MDT Coordinator for discussion at the Multi-Disciplinary Team (MDT) Meeting and to ensure that the MDT Coordinator receives all relevant up to date information for the patients to be discussed in a timely manner with awareness of 'cut off' dates/times.


Job responsibilities

The post holder will be responsible for tracking patients who have been urgently referred to the Trust from General Practitioners (GP) or General Dental Practitioner (GDP) who are suspected to have cancer, upgraded onto a 62 day pathway or have entered the system via a Screening pathway. Ensuring these patients are entered to the Somerset Cancer Registration (SCR) IT monitoring system.

The post holder will be responsible for tracking and monitoring all referrals against national cancer targets.

To accurately collect and record cancer pathway related data in the SCR cancer tracking system.

To extract information from other sources and Trust IT systems (i.e. Pathology, Radiology, Endoscopy, iPM etc.) to support the accurate, detailed recording and tracking of cancer pathways for patients on SCR.

To work closely with MDT Coordinators and Patient Pathway Coordinator teams to ensure that data collected on each patient on SCR is accurate and tracked on a daily basis.


Person Specification


Education and qualifications

* Good general education with English and Maths to GCSE standard or equivalent
* Evidence of IT skills and Healthcare information systems - knowledge of working with MS office, especially Excel and iPM or equivalent


Experience and knowledge

* Experience working in administration/secretarial capacity in a healthcare setting
* Experience of using computerised data systems
* Knowledge of medical, anatomical and oncological terminology
* An understanding of the current national issues affecting cancer
* Understanding of patient pathways


Skills and abilities

* Good attention to detail and methodical in approach
* Excellent interpersonal and communication skills and the ability to motivate, influence, inspire and encourage different and new ways of working
* Able to prioritise and work on own initiative and as part of a team
* Good time management and ability to work under pressure
