We have an exciting opportunity for a Personal Advisor to join our client supporting young people as they transition from care into adulthood. Location: County Hall, Worcester, WR5 2NP Rate: £19.21 per hour (Umbrella) Contract: Initial 4-month contract, with the possibility of extension Hours: 37 hours per week | Monday to Friday About the Role: As a Personal Advisor, you will be the key professional providing and coordinating support for care leavers (relevant and former relevant). You will play a vital role in helping young people achieve independence, stability, and their individual goals as set out in their Pathway Plans. Key Responsibilities: * Provide advice, guidance, and practical support to care leavers * Contribute to assessments and the preparation, review, and reassessment of Pathway Plans (post-18) * Design and deliver targeted interventions to support young people leaving care * Ensure the effective implementation and coordination of Pathway Plans * Work with partner agencies to ensure care leavers access appropriate services * Monitor and support young people’s progress, wellbeing, and outcomes * Maintain accurate, up-to-date case records, including financial support provided * Record all activity in line with Directorate recording policies (manual and electronic) * Actively challenge discriminatory behaviour and report concerns appropriately * Immediately report safeguarding concerns, including neglect, injury, or welfare risks, to the Team Manager Alternatively, if you know someone who may be interested, feel free to share this information
