You will work closely with multidisciplinary teams across community, primary care, acute neuro rehab, stroke early supported discharge, and other external agencies. The role is based at Hinchingbrooke Hospital, Huntingdon, with a requirement for county‑wide travel and independent driving.
Responsibilities
* Work autonomously in the community setting under the direction of a registered practitioner within the integrated multi‑professional ESD and neuro rehab team.
* Support patients on the ESD pathway in a ward environment, working closely with the ward multidisciplinary team and within the ESD/CNRS.
* Assess patients’ physical and functional needs, and plan and implement therapy interventions for stroke and neurological problems.
* Use clinical judgement to evaluate patient progress, recording objective and subjective measurements.
* Manage your own caseload of patients requiring ongoing support with complex needs, planning and prioritising workload.
* Undertake duties delegated and supervised by a registered professional, following prescribed packages of care.
* Keep the patient at the centre of care, ensuring patient and carer participation in decision‑making.
Requirements – Essential
* Full UK driving licence.
* Access to a personal car for independent travel across the county.
* Ability to travel independently around the county and meet strict time deadlines.
* Experience or training in neuro rehabilitation preferred (not mandatory).
