Overview
We are offering an exciting and unique new role as a Band 3 Recovery Worker within our Integrated Mental Health Teams (IMHT) across South Staffordshire, supporting our new First Transitions Pathway.
The First Transitions Pathway is a new service to help support individuals with complex mental health needs as they move from the structured support of forensic services to the community-focused care offered by our Integrated Mental Health Teams. This transformation is underpinned by a central commitment to promoting sustained recovery, fostering meaningful integration within the community, and empowering service users to lead fulfilling lives. This specialised team will work in close collaboration and partnership with the six IMHTs spanning South Staffordshire.
Responsibilities
* Assist in the formulation, implementation and evaluation of programmes of care for individuals carrying out all relevant forms of care under agreed levels of supervision
* Assist in engagement with services and identify strategies to support mental wellbeing and the promotion of self-care and social inclusion
* Communicate and work collaboratively with First Transitions team colleagues and Integrated Mental Health teams in the area to meet the needs of the service users

Person Specification
Qualifications
* Basic knowledge of mental health condition and rehabilitation
* Willingness to undertake Certificate in Mental Health/Social Care NVQ Level 2 (or equivalent)
* GCSE in Math and English Grade C or above or equivalent
Desirable
* Certificate in Mental Health - NVQ Level 2
Experience
* Knowledge and experience of working with people with or living experience of severe mental health problems
* Willingness to work with carers and support groups
Desirable
* Knowledge of promoting independence
* Knowledge of autonomous and team working
