Join Our New 24/7 SPUR Service as a Clinical Specialist Triage & Facilitator Nurse
At Birmingham Hospice, our patients are at the heart of everything we do. We are launching our new 24/7 Specialist Palliative Urgent Response (SPUR) service, which will operate between 8:00 PM and 8:00 AM, responding to urgent palliative care needs across Birmingham and Solihull.
As a Clinical Specialist Triage & Facilitator Nurse, you will be the first point of contact for all incoming calls to the service. A clinical leader, you will work autonomously and collaboratively with other healthcare professionals to assess, diagnose, and treat patients in their place of residence, ensuring timely and appropriate clinical responses.

* European Certificate in Essential Palliative Care (or working towards)

Job purpose
The Specialist Palliative Response Service (SPRS) will be delivered between the hours of 20:00–08:00.
During these hours the Triage/Facilitator Nurse will respond to urgent palliative response calls from patients, their families and professionals within Birmingham and Solihull, providing specialist palliative and end-of-life support and advice (via delegation to satellite teams or in person). Visits aim to take place within a 2-hour timeframe.
The role will require you to:
* Act as an autonomous practitioner, working independently and with other health professionals.
* Assess, diagnose and treat conditions in patients’ homes and initiate referrals as appropriate.
* Independently prescribe appropriate medication, evaluate or refer to other specialists as necessary.
* Work collaboratively with hospice, community, primary care and acute teams to meet patient needs and support policy delivery and clinical leadership.
* Provide expert professional advice to patients, carers and colleagues and maintain clinical excellence.
* Coordinate staffing resources and deliver priorities with heads of service; provide education and training to staff and students.
Key responsibilities
* Prioritise and triage patients with palliative, End-of-Life and acute conditions, making referrals as appropriate
* Implement and evaluate personalised care planning, advanced care planning and related processes (e.g., MASC, RESPECT)
* Provide specialist palliative care in the patient’s home as required, in line with clinical evidence, NICE and NSF
* Be sensitive to changing needs (physical, psychological, spiritual)
* Maintain accurate clinical records in line with legislation
* Support clinical emergencies and safeguarding matters per local and national policies
* Delegate clinical responsibilities safely within scope of practice
* Coordinate with external services and community teams to ensure ongoing support
* Lead and support the nursing team with resource planning and development
* Deliver education and training to staff and students
