We have a full‑time Band 5 community staff nurse vacancy within the City District Nursing Team, created due to the promotion of the current post holder.
As a Community Nurse you will develop advanced clinical skills in a range of areas, with no two days the same. Your base will be in Belmont, sharing a large office with neighbourhood team therapists to support excellent communication around shared patient care and concerns.
Main duties of the job
* Provide care to housebound patients and run a District Nurse Clinic for non‑housebound patients requiring specialised District Nurse skills.
* Support patients toward self‑management, enabling them to live as fully and productively as possible through education, equipment and lifestyle changes.
* Offer end‑of‑life support in close liaison with Hospice Teams and Specialist Palliative Care nurses.
* Collaborate with other health and social care professionals, care homes and care agencies.
* Record all work electronically on EMIS, an electronic patient record system that links with GP services; training and ongoing support will be provided to use EMIS and other necessary electronic systems.
