Overview

We are seeking a dedicated and compassionate Wound Care Nurse to join our healthcare team. The ideal candidate will possess a strong background in patient care. The Wound Care Nurse plays a crucial role in providing specialised care to patients with acute and chronic wounds. This position requires a compassionate and skilled nursing professional who can assess, plan, implement, and evaluate wound care treatments. The Wound Care Nurse will work closely with patients and healthcare teams to ensure optimal healing and recovery.

Hours: 15 hours per week


Main duties of the job

The post holder will undertake wound assessments and treatment for patients with both simple and complex wounds. Patients referred to this service will be the following types:

* Postoperative including suture and staple removal
* Post-trauma skin tears, self-harm
* Cavity wounds
* Burns
* Patient education including nutrition in relation to wound healing and scar management

There is an expectation to refer to the wider community team if required, support senior clinicians and HCAs, and attend meetings when required. The postholder will help promote the service and implement change as requested from the ICB.

Job responsibilities

DUTIES AND RESPONSIBILITIES

* Maintain good standards of communication both in written and oral form.
* Assess a wound and make appropriate dressing selections for the desired plan of care within local formulary and national guidelines.
* Prescribe safely within the prescribing competency framework and undertake prescribing audits relevant to service.
* Assess patients autonomously whilst remaining within scope of practice.
* Show awareness of evidence-based practice in wound care.
* Promote and implement services to new sites when required by the ICB for the expansion of the service.
* Make entries on patient records to record clinical interventions.
* Record in the clinical notes using the service area computer programme AdAstra.
* Complete associated documents to accompany specimens using Cyberlab where available and label specimens.
* Work well within a small team.
* Be flexible with working pattern across current locations to assist with the team’s annual leave/sickness coverage.
* Communicate with patients and carers in a polite and pleasant manner.
* Receive and deliver patient-related information to and from other departments via telephone/emails.
* Conduct/partake in relevant audits to improve service provision.
* To ensure mandatory training is up to date.
* Be aware of safeguarding issues and refer to local social services as appropriate. Staff should be trained to level 3 safeguarding through mandatory training.
* Keep updated on practice through educational events and guidelines both local and national.
* Acknowledge and accommodate barriers to communication and understanding including speech, hearing, language, and emotion.
* May be required to encourage patients to comply with programmes of care.
* Initiate emergency action when required.
* Inform the Clinical Manager of any untoward incidents and work safely under the clinical governance framework.
* Dispose of clinical waste in a safe manner.
* Contribute to infection control measures by adhering to infection control policies/procedures.

Education and In-Service Training

Be fully conversant in all the clinical computer systems used within the Wound Care Service.

Attend relevant study/induction days, seminars, courses, self development, etc. for individual development and for the benefit of the service. Keep up to date with mandatory training.

Where necessary, relevant training in the operation of new or unfamiliar equipment, software or procedures will be provided or arranged.


Confidentiality

In the course of your duties, you may have access to confidential information about patients, staff or health service business. On no account must such information be divulged to anyone who is not authorised to receive it. Confidentiality of information must be always preserved whether at or away from work. FCMS has in place a Whistleblowers Policy for staff wishing to express concerns.


Data Protection Act, 1998

Carry out any requirements within the duties applicable to the Data Protection Act, 1998.


Health and Safety At Work Act, 1974

Observe all responsibilities and carry out all duties, whether general to all employees or specific to the post, relating to Health and Safety in accordance with the Practice and Departmental Safety Policies and any statutory requirements.


Clinical & Corporate Governance/Quality Assurance

All employees are expected to comply with the clinical and corporate governance arrangements of FCMS. Every employee is personally responsible for the quality of the work, and standard of care, which they individually provide. It is their duty to seek to attain the highest standards achievable both individually and collectively within their knowledge, skills and resources available to them.

If you have other work or outside interests, this must not conflict with your duties and responsibilities of your attendance for work as an employee of the Practice.

It is a condition of appointment that you must inform your manager before taking up any private practice, work for outside agencies or other employers, other work for this Practice (including bank work), voluntary work or outside interests you have or propose to have. This is to ensure there is no question of it creating a conflict of interest with your NHS duties. You must also therefore seek your manager's approval before taking on any such other work or outside interest at any time after entering employment.


Working Time Directive

You are required to comply with the regulations governing working time and to any locally agreed associated arrangements.
