Family Support Worker Positive Futures Charity Jobs NI (part of the Happy Jobs group) is exclusively partnering with Positive Futures to recruit a dedicated, compassionate Family Support Worker in Omagh. This rewarding role supports children and young people with a learning disability and their families to build resilience, access their community and achieve their goals. About Positive Futures: Positive Futures supports people with learning disabilities, acquired brain injuries and autism to live fulfilling, independent lives through exceptional, person-centred support. About the Role: As a Family Support Worker, you'll provide practical and emotional support to families, coordinate person-centred plans, and work closely with Trust partners and community services to deliver agreed outcomes. Travel will be required across Omagh and surrounding areas. Key Responsibilities: Person-Centred Support: Build positive relationships, complete and review person-centred plans and risk assessments, and provide tailored support to meet agreed outcomes. Family Resilience & Community Access: Help families navigate services, participate in social and recreational activities, and integrate into their local community. Advocacy & Safeguarding: Champion the rights of children and families, maintain confidentiality, and follow safeguarding procedures. Partnership Working: Liaise with professionals, attend team/statutory meetings, contribute to service quality and participate in the on-call rota. Qualifications & Experience: Essential: Minimum 2 years' professional experience closely supporting families or individuals through challenging events; right to work in the UK; full driving licence or ability to travel independently to meet the requirements of the role. Desirable: Advocacy/negotiation experience; knowledge of person-centred planning tools; community networking and partnership working; mediation/conflict resolution; creative, solution-focused practice.
